Pennsylvania offers Home and Community-Based Services (HCBS) Waivers to support individuals who prefer long-term care in their homes or communities instead of institutional settings. 3. Role of Support Coordinators
County MH/ID offices are crucial in determining eligibility for services. If you are found eligible, a supports coordinator will be assigned to you to:

- Assist with your application for the Medicaid Home and Community-Based Waiver Program
- Enroll you in necessary services
- Help develop your personalized support plan
- Connect you with community agencies and resources, including EmPowered Horizons
- Coordinate and oversee your support services.
